This page allows you search the Arizona directory for chiropractors and view details regarding disciplinary actions1 licensing, addresses and phone numbers.

 

To find a State of Arizona Chiropractor:

  1. Select a category to search.

  2. Type a search criteria for the category such as a last name (or beginning letters ) license number, or city.

1  Only disciplinary actions are shown in the search results. The Board office should be contacted to determine if the licensee has a history of dismissed complaints, advisory letters or non-disciplinary orders.

Furthermore, the presence of an individual listed in this directory does not in any way constitute an endorsement by the Board of Chiropractic Examiners.

All Chiropractic licenses, unless renewed, expire on December 31st every year.  An ACTIVE status means a license is not expired.  An Administrative Suspension (ADM SUSPENSION) status is imposed when a licensee fails to timely and accurately renew their license.
